A Clinical Study on Trochanteric Fracture of the Femur


Abstract
In treatment of trochanteric fractures of the femur, reduction in stable position and rigid internal fixation is required to decrease mortality and morbidity by early ambulation and early weight bearing, especially in elderly. In treatment of unstable fracture, early amb¡þulation and early weight bearing is possible by medial displacement and internal fixation in valgus position with satisfactory result.
From Jan. 1973 to Mar. 1978, 50 trochanteric fractures were treated by surgical mea-sure. at orthopaedic surgery department, Chungnam National University Hospital, and following results were obtained.
1. Of 50 cases of trochanteric fractures, 36 cases were male, 14 cases were female.
2. In male the incidence was relatively even over all age groups, but in female most of them occurred in elderly patient over 60 years old.
3. The most common cause of fracture was traffic accident in all and in male, but in female slip down was.
4. Unstable type was 62% and comminuted type was most common.
5. In unstable type, stable reduction and rigid fixation was achieved by medial displacement and internal fixation in valgus position with satisfactory result.
6. The incidence of early complication was markedly reduced by early ambulation.
7. The most common late complication was coxa vare deformity and it was mainly noticed in unstable fracture reduced in anatomical position.
